When Your Child Needs Hospital Admission



 

If your child is being admitted to Saint Christopher’s Hospital for Children, we will need the following information:

 

  • Patient name
  • Name of parents/guardians
  • Address of patient and parents/guardians
  • Telephone number of parents/guardians
  • Dates of birth of patient and parents/guardians
  • Social Security number of patient and parents/guardians
  • Diagnosis
  • Type of insurance or medical assistance coverage
  • Isolation requirements, if necessary
  • Referring and family physician information
  • Emergency contact information
  • Co-pays/deductibles

 

Please bring with you the following documents:

  • Photo ID of parent/guardian
  • Insurance card(s)
  • Custody papers if applicable

 

A a parent or guardian must accompany the child at the time of admission to ensure that proper consent may be obtained for treatment. If a parent or guardian is not present at the time of an elective admission, the admission may be postponed until proper consent is obtained.
